First time visiting Prevail Prosthetics and Orthotics? Welcome!
For your initial appointment, please bring the prescription from your physician (they may have already faxed it to us), your insurance card(s), and a photo ID for the patient or responsible party. We’ll verify your insurance benefits before your visit.
After your evaluation, we’ll create a personalized plan of care and review any out-of-pocket costs with you. When you’re ready to proceed, we’ll handle the authorization process with your insurance provider(s).
